Opt-In Form
Submitting your Opt-In Form is the only way to receive a payment from the Hanes, et al. v. Thumbtack, Inc. settlement. Verify your eligibility with the Notice ID and PIN from your notice, then sign your form. Watch the deadline shown on the home page.
